What is a Continuity of Care Document (CCD)?

The CCD is a global standard for electronically exchanging a patient health summary. The CCD contains a core data set of the most relevant administrative, demographic, and clinical information about your or a family members healthcare. Covering one or more healthcare encounters, it provides a means for one physician, hospital, or other clinical setting to aggregate all of your pertinent data and electronically forward it to other healthcare providers, to support the continuity of care.
Its primary use case is to provide a snapshot in time for a specific patient. These "snapshots" of a patient's health data that are useful and possibly lifesaving, if available at the time of a clinical encounter.
Because it is expressed in the standard data interchange language known as XML, a CCD can potentially be created, read and interpreted by any electronic medical record (EMR) or electronic health record (EHR) software application.
